Here’s how it works:

Ready to Get Insight Into Your Health?

Contact our office at (920) 499-1333 to request a Patient Symptom Survey (PSS) to be sent to you via email.

Once we receive your PSS, one of our doctors will review your responses. Based on your unique health concerns, our doctors will then determine what labs will give us the best insight into your health. We will email you with their Insight Functional Health Testing recommendations, including the cost of each test. When you are ready to proceed with testing, simply call our office to order your lab testing.

Once your testing is complete, Dr. Susan Frain or Dr. Patrick Frain will review your test results. Our office will contact you to schedule a time to sit down with the doctor to review your lab results, and receive recommendations to help you improve your overall health.

Recommendations may include exercise, personalized diet/nutrition recommendations, and/or specific supplement recommendations including daily amounts to take.
